Healthcare Reform and Operating Room Efficiency

Don’t believe the media. Obamacare is already adding to the unemployment rolls.

Commenting on the mainstream media of his day, Thomas Jefferson famously observed that "The man who reads nothing at all is better educated than the man who reads nothing but newspapers." Coming from the Sage of Monticello, whose credentials as an advocate of the free press were pretty darned solid, this is an arresting statement. And it is evident that the news business has changed little during the two centuries that have come and gone since he penned that uncivil passage. Even if one ignores the "reporting" perpetrated by the New York Times and other leading lights of journalism in the wake of the Tucson shooting, the continuing relevance of Jefferson’s comment can be verified by perusing the stories appearing in most prominent publications about the GOP effort to repeal Obamacare .

The new House majority incurred the wrath of the media by refusing to change the name of the repeal bill and ignoring their calls to "wait a bit longer" before bringing it up for a vote. In fact, the Republicans had the temerity to give the "Repealing the Job-Killing Health Care Law Act" pride of place in their vote schedule. This affront has inspired a spate of "news" stories claiming that the GOP has misled the public concerning Obamacare’s effect on unemployment. The Republicans have, according to these articles, misrepresented a CBO report that suggests the new law will eliminate 650,000 jobs. A typical example is an AP "fact check" piece that includes this priceless quote from economist Paul Fronstin: "CBO isn’t saying that there is job loss as much as they are saying that fewer people will be working."

The irony of such verbal contortions is that they would be unconvincing even if they were not self-refuting. It isn’t necessary to invoke the CBO to demonstrate that Obamacare is a job killer. The ink was hardly dry on the new law when the layoffs began at the small insurance companies. Last year, HealthMarkets announced its plan to eliminate 250 jobs pursuant to "national healthcare reform." Likewise, Principal Financial Group said it would get out of the health insurance business and lay off 1,500 workers due to "the shadow of a lot of regulatory matters." The darkest of these regulatory shadows is Obamacare ‘s requirement that health carriers spend 80% of premiums on medical care. This mandate will drive many other carriers out of the market as well, and kill the jobs of anyone working in their health divisions.

The job losses have by no means been limited to health insurance companies. The actual providers of health care, including community hospitals, are beginning to eliminate jobs. Memorial Hospital in South Bend, Indiana, has had to cut nearly 50 jobs because Obamacare "has already started to cut reimbursement rates to hospitals across the region and the country and we expect that reality to get worse moving forward." Another small hospital, in Leominster, Massachusetts, will eliminate about 50 positions. The spokesman for that institution said the cuts were made necessary by "health care reform, with its reductions in Medicare and Medicaid reimbursements." Yakima Valley Memorial, in the state of Washington, will also cut 70 to 100 jobs because "Under federal health care reform, Memorial must reduce its expenses."

In addition to the jobs it will destroy in the insurance and hospital industries, Obamacare ‘s new taxes will kill countless jobs in the medical device industry. The Massachusetts Medical Device Industry Council estimates that about 90 percent of member firms will be forced to "cut back on operational costs — and jobs — after [Obamacare ‘s] 2.3 percent tax on their products is implemented in 2013." And, lest you fall into the error of thinking that "reform" will bring enough new customers to offset the increased tax burden, 42 percent of the affected firms have indicated that Obamacare will not increase their business. The others are no doubt hoping that at least one of the promises made on behalf of Obamacare will actually be fulfilled. Inevitably, however, this addition to their cost of doing business will add to the unemployment rolls.

If you believe that all this carnage will somehow render health care more "efficient," as many progressive policy wonks have claimed, remember that reform-induced job losses will not be limited to the health care industry. As health care economist John Goodman points out, the various mandates imposed by Obamacare will effectively raise the cost of labor across the economy. He estimates that they will add ".28 an hour for full time workers (individual coverage) and .89 an hour (family coverage) for fulltime employees." Many businesses simply cannot absorb such increases without cutting labor costs. This is why we have heard so many cris de coeur from low-margin employers like White Castle, which says the financial hit "will make it hard for the company to maintain its 421 restaurants, let alone create new jobs."

And the government cannot simply "waiver" this problem away. Thus far, the Obama administration has issued get-out-of-jail-free cards to well over 200 employers and unions that screamed bloody murder when they realized how much devastation Obamacare will wreak on their operations. These waivers don’t cure the disease. They merely treat its symptoms. Even worse, they will eventually draw U.S. health care into an increasingly corrupt, Chicago-style political spoils system. Because these waivers are issued at the pleasure of the HHS Secretary, it is inevitable that the companies and unions with the most political power (i.e. those which have made the largest cash donations to the Democrats) will be given special dispensations. Less affluent employers will be forced to save money by cutting their labor costs.

None of the "reporters" peddling the CBO meme have shown any interest in the job losses already caused by Obamacare . They prefer to focus on the wickedness of the Republicans. As Paul Krugman phrases it, "The key to understanding the G.O.P. analysis of health reform is that the party’s leaders are not, in fact, opposed to reform because they believe… it will be ‘job-killing’ (which it won’t be). They’re against reform because it would cover the uninsured — and that’s something they just don’t want to do." This is the sort of dishonesty that prompted Jefferson to augment the above-quoted criticism of the Fourth Estate with the following: "Advertisements contain the only truths to be relied on in a newspaper." Were he with us today, he would no doubt include broadcast media and many Internet outlets in this indictment.

MIAMI — As federal health officials are aggressively courting young adults to sign up for health insurance with celebrity endorsements and social media campaigns, they are also getting significant help from the very demographic they’re targeting.

Busy medical, nursing and law students across Florida are getting certified as counselors and are staffing enrollment events as the March 31 deadline to sign up for insurance under the Affordable Care Act looms. Many of the students were active in outreach programs to provide medical and legal services in low-income neighborhoods, but being “able to sign up patients for health insurance and get coverage that’s more than just one time care really completes the circle,” said Ali Moody, a second year medical student at the University of Miami Miller School of Medicine.

More than 60 UM medical students went through a five-hour training to become certified application counselors and have enrolled more than 50 people since December. They’ve staffed several major enrollment events as well as manned tables daily outside their school, where they end up enrolling many low-income patients recently released from the nearby hospital.

University of Miami’s medical school places a strong emphasis on connecting students to underserved communities “so it’s natural to have them take an active role in getting the same patients enrolled in the Affordable Care Act so they get preventive care more frequently and keep them out of the emergency room,” said Donna Shalala, the school’s president and former Health and Human Services Secretary under President Bill Clinton.

Farther south at Florida International University in Miami, seven students were crammed around a massive conference table this week trying to sign up for health insurance on their laptops.

Law students Allan Zullinger, 28, and Anthony Rouzier, 27, hustle back and forth across the room, overseeing two and three enrollees at a time, all in various stages of the application process, explaining to a professor and his wife that they must use next year’s tax income to determine if they are eligible for a subsidy and telling a student that they may qualify for insurance under their parents’ plan because they are under the age of 26.

Rouzier frequently peers over Valentina Adarraga’s laptop, cheering her on, getting on the phone with federal health officials when her application stalls and explaining various health plans during the nearly three-hour long process.

The 20-year-old ultimately chose a silver plan with a monthly premium, thanks to a 9 tax credit. Adarraga, a full-time student, also works part-time as a beach attendant.

“It’s good to know I’m not going to be denied assistance,” said Adarraga, who has been without insurance for several months and recently couldn’t get treatment when she tried to see a doctor for serious migraines. She tried signing up on her own online, but wasn’t able to complete the process.

“We understand that it’s going to take people like us to make it work,” said Zullinger, who works more than a dozen hours a week enrolling people. “It’s an experiment almost and we understand that we have to put the labor in right now.”

WHO BENEFITS FROM OBAMACARE? DEMOCRATS, REPUBLICANS & THE CORPORATIONS THAT

In last week’s United States Supreme Court ruling re National Federation of
Independent Business Et Al v. Sebelius, Secretary of Health and Human
Services, Et Al (11-393/28-Jun-2012) a.k.a. "NFIB v. Sebelius", the
individual mandate to purchase health insurance and associated penalties for
non-compliance contained in the Patient Protection and Affordable Care Act
(Public Law 111-148/H.R.3590/PPACA/23-Mar-2010) a.k.a. the "Affordable Care
Act" a.k.a. "Obamacare" were declared to be a tax and therefore
constitutional:

The Obamacare bill is a massive piece of legislation – depending on
formatting, anywhere from 900+ to 2000+ pages – much of which can loosely be
construed as "healthcare reform". That millions of Americans will be
impacted by this law is beyond dispute. Whether that impact will be
positive or negative, on the other hand, will be a source of sociopolitical
debate for many years – assuming of course our illegal and immoral
death-from-above droning and Oil Wars Iran-baiting antics in Syria,
Baluchistan and elsewhere don’t get us all blown to Kingdom Come in the
interim:

There are, however, three clear and immediate beneficiaries of Obamacare:
the Democratic Party, the Republican Party, and the Global Corporations who
control them both:

1. THE DEMOCRATIC PARTY can truthfully energize their base of lemmings on
the left side of the false divide by declaring a victory for "healthcare
reform" – although the percentage of Americans who can expect to receive a
net benefit from all the changes in that mass of legalese is only about 10
percent, and some 5 million of us are projected to actually LOSE our health
insurance coverage as a result of Obamacare. According to the Congressional
Budget Office (CBO):

"Compared with prior law, the ACA is now estimated by CBO and JCT to reduce
the number of nonelderly people without health insurance coverage by 30
million to 33 million in 2016 and subsequent years, leaving 26 million to 27
million nonelderly residents uninsured in those years… The share of legal
nonelderly residents with insurance is projected to rise from 82 percent in
2012 to 93 percent by 2022. According to the current estimates, from 2016
on, between 20 million and 23 million people will receive coverage through
the new insurance exchanges, and 16 million to 17 million people will be
enrolled in Medicaid and CHIP. Also, 3 million to 5 million fewer people
will have coverage through an employer compared with the number under prior
law."

2. THE REPUBLICAN PARTY can legally (and from that narrow perspective
truthfully) energize their base of lemmings on the right side of the false
divide by parroting Chief Justice Roberts’ proclamation that its individual
mandate and therefore Obamacare represent a "tax increase" from a President
whose campaign promised no tax increases on the middle class, and who
personally and through his administration vowed repeatedly that Obamacare
was NOT a tax. In Obama’s own words:

0,000 a year will see any form of tax increase. Not your income tax, not
your payroll tax, not your capital gains taxes, not any of your taxes."

3. THE GLOBAL CORPORATIONS that control both the Democratic and Republican
Parties – and through that "Two-Party Tryanny" our government and all of us
– are the biggest beneficiaries of Obamacare. The global fascist elite 1%
could care less about improving the health of the other 99% or what new tax
burdens are placed on the backs of a middle-class struggling to survive yet
still desperately hanging on to the belief that somehow, someday they too
will be rich, famous and have their own Reality TV show if they just stay on
the treadmill and keep running. And by using the Congressional power to tax
rather than the Commerce Clause as the basis for his decision, corporate
puppet Chief Justice Roberts made sure the NFIB v. Sebelius ruling would
stir up a partisan political firestorm – and accompanying smokescreen – to
keep the Sheeple divided and fighting each other, blinded to the
white-collar crime being perpetrated by their common enemy:

Reaching far beyond the corporatization of medicine and privatization of
healthcare in America, the 1%’s objective with Obamacare was to establish a
constitutional legal precedent for individual mandates by which the wealthy
can become even wealthier simply by having their paid political puppets pass
laws mandating that we buy their corporate products and increase their
profits – regardless of whether we want, need or can afford them.

he won’t get it from Barack Obama (a.k.a. Odrona the Indefinite Detainer),
and he won’t get it from Mitt Romney (a.k.a. the Godfather of Obamacare)…

Affordable Care Act on June 28, voters are guaranteed to continue hearing
the same old false claims about the law from politicians. And President
Barack Obama and presumptive GOP presidential nominee Mitt Romney wasted
little time in taking to the airwaves to rehash plenty we’ve fact-checked
before. Obama even threw in a few new claims.

* Obama reiterated his "if you like your plan, you can keep your plan"
refrain, despite the fact that at least a few million workers won’t keep
their employer-sponsored plans, according to the Congressional Budget
Office.

* The president also exaggerated the benefits of the law, such as the number
of young adults who were able to join their parents’ plans, thanks to the
law, and the number of individuals who will receive rebates issued by
insurance companies that didn’t spend enough premium dollars on health care.

* Romney repeated a number of distortions, saying that the law would "cut
Medicare" by 0 billion and that it "adds trillions to our deficits."
That’s a reduction in the future growth of Medicare spending over 10 years.
And CBO says the law would reduce the deficit.

* Romney said the law is a "job-killer." But CBO says the law would have a
"small" impact on jobs, mainly affecting the amount of labor workers choose
to supply. Those getting subsidies, for instance, might work less hours
since they’re paying less for health care.

* Romney claimed the law "puts the federal government between you and your
doctor." The law would set minimum benefits packages, but medical services
will not be government-run, nor does the law allow for rationing of care.’

Average Joe won’t get the plain and simple truth about what the SCOTUS
Obamacare decision really means from Congress, from TV coverage, or from the
corporate-sponsored political puppets representing either side of the
"Two-Party Tyranny" the global elite use to keep us divided and fighting
each other rather than united and attacking them. Fortunately, however, he
can get a glimpse of it here:

"A deeply divided Court held that the individual mandate, which requires
that virtually all Americans either obtain health insurance or pay a penalty
by 2014, is constitutional… Defending the constitutionality of the
mandate, the government’s primary argument was that Congress can require
everyone to buy health insurance using its power under the Commerce Clause
of the Constitution, because the failure to buy insurance shifts the costs
of health care for the uninsured to health-care providers, insurance
companies, and everyone who does have health insurance. Five Justices – the
Chief Justice and Justices Kennedy, Scalia, Thomas, and Alito – all rejected
that argument. But the government still won, because a different set of

The bottom line is this:

As were "Citizens United v. FEC" (which allows global and even foreign
corporations to buy our elections) and "AT&T Mobility v. Concepcion" (which
makes consumer class actions against corporations infinitely more
difficult), the SCOTUS "NFIB v. Sebelius" decision is a BIG WIN for
corporations in general and health care and insurance companies in
particular. Contrary the the claims of clueless Obamapologists from coast
to coast, neither Obamacare nor this ruling "paves the way to Universal
Healthcare Coverage". Instead, they set the precedent for more individual

the Democratic Party, the Republican Party, and the Global Corporations who
control them both:

1. THE DEMOCRATIC PARTY can truthfully energize their base of lemmings on
the left side of the false divide by declaring a victory for "healthcare
reform" – although the percentage of Americans who can expect to receive a
net benefit from all the changes in that mass of legalese is only about 10
percent, and some 5 million of us are projected to actually LOSE our health
insurance coverage as a result of Obamacare. According to the Congressional
Budget Office (CBO):

"Compared with prior law, the ACA is now estimated by CBO and JCT to reduce
the number of nonelderly people without health insurance coverage by 30
million to 33 million in 2016 and subsequent years, leaving 26 million to 27
million nonelderly residents uninsured in those years… The share of legal
nonelderly residents with insurance is projected to rise from 82 percent in
2012 to 93 percent by 2022. According to the current estimates, from 2016
on, between 20 million and 23 million people will receive coverage through
the new insurance exchanges, and 16 million to 17 million people will be
enrolled in Medicaid and CHIP. Also, 3 million to 5 million fewer people
will have coverage through an employer compared with the number under prior

2. THE REPUBLICAN PARTY can legally (and from that narrow perspective
truthfully) energize their base of lemmings on the right side of the false
divide by parroting Chief Justice Roberts’ proclamation that its individual
mandate and therefore Obamacare represent a "tax increase" from a President
whose campaign promised no tax increases on the middle class, and who
personally and through his administration vowed repeatedly that Obamacare
